Medela Symphony Plus = $79 monthly (no sales tax)

Medela Lactina Select = $59 monthly (no sales tax)

One way Fedex Ground shipping = $45

1.) First, CALL (702) 862-6435 to confirm availability and place pump on 24 hour hold.

2.) Download the Sym rental Agmt pdf here

3.) Fill in all required information.**

4.) Sign and date BOTH pages of the rental agreement.

5.) Return the signed agreement within 24 hours, by Fax (1-509-463-8512) or email to info@angelpumps.org

6.) Call (702) 862-6435 between 11am & 8pm Pacific time to supply your credit card or debit card information.

7.) Tracking information will be sent by email after your order ships.

** If you prefer to receive a completed rental agreement, please complete step one and then send an email with the following information:

• Your Full name

• Partner name

• Full address

• Two phone numbers

• Email address

• Date of birth

• Drivers License ID# (and state of issue)

• Do you need to purchase a Medela Double pumping kit?

After you receive the rental agreement (sent to you by email), please complete steps 4 - 6 above.

 

Important:

• Due to high demand, rental pumps can only be held for 24 hours.

• All rentals will AUTOMATICALLY renew month to month, until equipment is returned.

• Return shipping (cost) is renter's responsibility.  Instructions supplied with rental.

• Rental pumps are shipped by Fedex Ground and a signature will be required upon delivery.

• Shipping time is 1-5 business days, see map (Continental U.S. shipping only)
